Overview

Under the Settlement, eligible Class Members who requested copies of medical records (either directly from a health care provider or through an attorney) who were billed by ChartOne on or after August 7, 1999 and paid more than the allowed amount may be eligible to receive a $15.00 payment from ChartOne, Inc. If you are an eligible Class Member and wished to have received this benefit, you or a Personal Representative must have submitted a Claim Form with proper documentation attached and postmarked no later than January 31, 2008 to the Class Administrator.

If the Court approves the Settlement at the Settlement Hearing, two lawsuits filed in the District of Columbia Superior Court alleging ChartOne, Inc. overcharged persons who requested copies of their medical records (either directly or through a lawyer) will be resolved without any actual admission of fault or wrongdoing on the part of ChartOne, Inc.

Class Member Options

Participate in the Settlement

If you are an eligible Class Member and you wished to have received a one-time payment of $15.00 under the Settlement, you or a Personal Representative must have completed and submitted a Claim Form with proper documentation attached and postmarked on or before January 31, 2008.

Exclude Yourself From the Settlement

The deadline for Class Members to exclude themselves was October 30, 2007.

Object to or Comment on the Settlement

The deadline for Class Members to object or comment on the Settlement was October 30, 2007.

Do Nothing

If you did nothing, you were automatically included in the Settlement Class and gave up your right to file your own lawsuit, but to receive the $15 you must have submitted a Claim Form by January 31, 2008.

Settlement Hearing

A Settlement Hearing was held before the Court on January 11, 2008 at 2:00 p.m., in Courtroom No. A50 at the District of Columbia Superior Court, to determine whether the proposed settlement should be approved. You did not need to attend the Settlement Hearing unless you objected to some or all of the terms of the Settlement.
